Insurance Information

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. Patients with insurance must realize that insurance is a product and while some are good, others are insufficient. In many cases, medical and/or dental insurance pays a portion of the investment in surgical care. Insurance is generally designed to assist you with the expense, not eliminate it.

The financial coordinator will assist you in determining your specific benefits and as a service to you, will file your insurance claim at no charge. In order that we might render this service in a more timely fashion, please be sure to bring all information regarding your insurance policy to your initial consultation or fill out the patient online form provided for you on our web site.

If you have any uncertainty as to the extent of your coverage, we recommend pre-determination as a method of determining benefits. This can be done at your initial consultation appointment.

We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.

We are a preferred provider for Regence Blue Shield, Premera Blue Cross, Delta Dental Service, MetLife, Cigna, Aetna, and Principal Financial Group.

For your convenience we accept Visa, MasterCard and CareCredit. We deliver the finest care at the most reasonable cost to our patients, therefore partial payment is due at the time service is rendered unless other arrangements have been made in advance.
